Sleep Apnea and Your CDL: What Drivers Need to Know

Sleep apnea screening has become more common. Here’s the driver’s guide.

What is Sleep Apnea?

A condition where breathing repeatedly stops during sleep, causing:

  • Poor sleep quality
  • Daytime fatigue
  • Potential safety hazard while driving

Why FMCSA Cares

Per FMCSA medical guidelines:

  • Sleep apnea increases crash risk
  • Estimated 20%+ of drivers affected
  • Often undiagnosed

Screening Indicators

Medical examiners may recommend testing if you have:

  • BMI over 35
  • Neck circumference 17"+ (men) or 16"+ (women)
  • Reported symptoms (snoring, daytime sleepiness)
  • Witnessed breathing pauses

The Sleep Study

If recommended for testing:

  1. Home sleep test or lab study
  2. Measures AHI (apnea-hypopnea index)
  3. Results determine severity

Treatment Options

CPAP

  • Most common treatment
  • Must show compliance (70%+ nights, 4+ hours)
  • Compliance data reportable

Other Options

  • Oral appliances
  • Weight loss
  • Positional therapy
  • Surgery (rare)

Impact on Certification

  • Untreated moderate/severe: May be disqualified
  • Treated with compliance: Usually certified
  • May require annual review
  • Must report treatment changes

Protecting Your Career

  1. Get tested if recommended
  2. Use treatment consistently
  3. Keep compliance records
  4. Don’t try to hide it

It’s manageable - don’t let it derail your career!
